Re: Mr.Henry  O ' Keefe

DOB: 2 February ,1925


I am writing to request you to arrange a home visit for Mr O' Keefe, who underwent removal of Mmalignant Mmelanoma large lesion, requires , further assessment as well as to arrange assistance in managing his Daily needs livings as his wife finds it difficult to manage by herself.  he lives with aged wife.


Mr O' Keefe got admitted to hospital on 14 March,2008 with 1- the diagnosis of Malignant Melanoma left shoulder and he underwent large lesion removal on the same day. He was discharged on 17 March , 2008.

At my first home visit on 18 March,2008  I assisted Mr Henry to shower and dressed his wound which was healthy. However, Mr O'Keefe experienced increased pain in the hands and legs. Therefore, he was given panadol  2 which was prescribed to him 3 times a day. Besides, he has been continuing  Glucosamine and  Chondroitin supplement as recommended.Mrs.O'Keefe has concerns that Mr O’keefe is showing sign of forgetting things which made her worried. In addition, she seeks the help of neighbours to assist her daily living activities. On my today's visit he was not remembering that I showered and dressed his wound yesterday. The house  was were filthy and disorganized. However, Our service will be continued till 24 th March and his suture will be removed at Master Public Hospital.

In view of the above, it would be highly appreciable if you could make an arrangement as soon as possible to get assistance for assist their daily livings.

Focus should be on their needs 

As soon as possible should be in the introduction
