Re : Mr Tom Riddle, aged 20


I ( space)am referring Mr Riddle ,who has asthmarequires your expert advise on identification of  his allergies and guidance regarding environmental management .

Mr Riddle currently  lives in a new accommodation  where he has  two cats and he owns an old dusty  carpet.In addition, he sleep on the floor.His past medical history revealed that  he has had child hood asthma and it was not shown any flare- up since 8 years .He has periodic eczema and he does not have any known allergies.

On  01 June,2018 Mr Riddle was presented in the clinic with the complaint of breathing difficulty since(for)2 weeks along with persistent cough, wheezing and itching on his eyes. On assessment, 2 his chest x ray was clear and bronchodilator test was positive. 1 He was initiated  the treatment (treatment was initiated) with steroids and anti asthmatic agents , also advised him about allergen management however his symptoms got worsened  in next few visits.

  • 1-importance should be given for the treatment 

  • Be careful with passive usage.

  • 2 - no need to write normal results

  • This could have been the second paragraph 



During today's visit , discussed with him regarding environmental triggers and  how to use inhalers properly.


  • Current information should be explained in detail 

  • an allergist should know about the current status and active problems



It would be greatly helpful id if you could provide necessary management for him.
