Re : Mr Henry O'Keefe

DOB: 02 February, 1925


I am writing to request an urgent home visit for Mr and Mrs O'Keefe to assess their needs and to arrange appropriate further assistance, as they have difficulties to cope with daily living activities.

Mr O'Keefe, an aged pensioner, who lives with his wife Dorothy. She also a/an aged pensioner.They does not have any children.

  • Need to improve sentence construction.


Mr O'keefe has been under our care, following his discharge from hospital on 17 March, 2008. He underwent surgery for the treatment of malignant melanoma on his left shoulder.1

During home visits, Mr O'Keefe has been provided wound dressings and assisted with showering. His wound is being was healed well without getting any complications. However, he has been developed impaired memory, unsteady gait, and rests most of the day. Therefore, Dorothy has difficulties to manage him, the house and the garden. Apart from that our care will end when his sutures to be removed on 24 March.
