Dear Sir/Madam,
Re: Ms Nicola Boyle, aged 20 (both mother and child should be referred)
I am writing to request a home visit for Ms Boyle who has given birth to her first child. She requires advice and assistance in for the care of the baby, following her discharge today.
Refer letter number 29 for introduction.
Ms Boyle underwent an emergency caesarean section on 11 September due to fetal distress and failure to labour. A male baby was born with 4.0 [check birth weight]kilograms. On At that time, his Apgar score was 6 in one minute and increased to 9 with the support of oxygen.

Postoperatively, Ms Boyle has developed post partum[one word] haemorrhage. Her haemoglobin level was 90gram/L; for which Fefol and Vitamin C were initiated.[*] She was not confident in breast feeding[one word]; therefore, preferred to change to bottle feed. Apart from that she was not confident in bathing as well as taking care of the baby. 1Currently, her blood loss is minimal, the wound is clean and dry. Discharge weight of the baby is recorded as 4.0 kilograms.

Ms Boyle is single and has no contact with the father of the child and her family members. Therefore, she feels very isolated and insecure. Moreover, she has doubts in about her ability to be a good mother and talked about offering her baby for adoption.
In view of the above, kindly visit Ms Boyle and her baby and provide care and assistance to them.
