Re: Ms Nicola Boyle (both mother and child needs to be referred)

Age: 20 years


I am writing to request you to provide  arrange a  home visit arrangement to for Ms Nicola Boyle  who is a new mother requires advice and assistance for herself and her newborn baby. as she is They are being discharge today.


Ms Nicola Boyle presented to hospital at 1600 hours on 10 th January,2012 with contacting 1:10 minutes.She has had fetal distress and failure to progress after her 16 hours of 1st Stage of Labour.Therefore,she has underwent Emergency Caessarean( spell check) Section.On 11th January,2012 she has given birth to male baby.The babies baby's birth weight was 4.3kgs and Apgar score was 6 at 1 minute,9 at 5 minute after the resusitation.(spell check)


Ms Nicola Boyle is a single mother and has no contact with the father of   llo child since  for 4 months.However,her Obstetrical  history reveals no Antenatal complications and had 8 Antenatal visits.


Ms Nicola Boyle's postnatal recovery was uneventful.She had 800 ml of immediate postpartum hemorrhage which is minimal now.Her hemoglobin on 12th January 2018 was 90g/l, therefore she was prescribed with Fefol( Iron supplementation) and vitamin C.She was started with breastfeeding . However,she is not confident to feed and bath her baby.In addition,she desires to change bottle feeding.Baby is feeding well and the current weight is 4.0 kg.
