Re:Ms Nicola Boyle (both mother and child should be referred)

I am writing to refer Ms Boyle,a single mother, who requires a home visit to provide proper advice and assistance for herself and her baby care. She is being discharged today.
On 10 January,2018, Ms Boyle presented to hospital with contraction of labour. She experienced 16 hours of labour; therefore, due to fetal distress and failure to progress, an emergency caesarean section was performed. She gave birth to a healthy baby boy.
Avoid writing information that is not in the case note.
During hospitalization., iron supplement and vitamin c were prescribed to for Ms Boyle. The baby started breastfeeding well and his current weight is 4.0kg.However.she is not confident in taking care of her baby.
However is not apt here.
Ms Boyle has no contact with the father of the child and her family. She lives in a rental share flat and is on sole parent benefit. Please note, she feels very isolated as well as insecure and is considering to give the baby for adoption.

Pertaining to the above information, it would be greatly appreciated if you could organize a visit to Ms Boyle and her baby at their residence. It is worthy to provide appropriate guidance and help to this family.

Good letter only thing you missed some information about the baby such as birth weight and apgar score.
