Re: Mr Henry O' Keefe 

DOB: 02 February,1925

Mr Henry O'Keefe is recuperating from malignant melanoma in the left shoulder. He and his wife require a home visit to assess their needs and to arrange arrangement for further appropriate assistance. 

Mr O'keefe was admitted to the hospital on 14 March,2008 where the large lesion was removed. Since his discharge from the hospital, he has been provided with wound dressing and assistance in showering from our service which will end in/on 24 March,2008. Presently, his wound healed well; however, he seems still confused and a little unbalanced. 

Mr O'keefe resides with his wife, Mrs Dorothy, they both are aged pensioners. During today's home visit, his wife conveyed her concerns regarding the management of the house and garden. At present, they receive help from their neighbours for cooking and shopping. 

Kindly organise a home visit as soon as possible to evaluate their requirements. It would be greatly appreciated if you could arrange an ongoing assistance in managing the house, garden, shopping and preparation of cooking. Their residential address is 12 Donaldson Street, Greenslopes 4121.
