Dear Sir/Madam,


Re: Ms Nicola Boyle, aged 20 (both mother and child should be referred)


I am writing to request [this style is a little bit awkward nowadays] a home visit for Ms Boyle and her baby. They require advice and assistance in baby care to maintain proper well-being. [Don't add anything that is not in the case note.]

On 10 January,2018, Ms Boyle was admitted to hospital due to contraction of labour. 2Her first stage of labour was 16 hours. Next day at 11.20 hours, she underwent an emergency caesarean section due to fetal distress and failure to progress. She gave birth to a 1healthy male baby.

During hospital stay Postoperatively, Ms Boyle had post partum[ postpartum is a one word] haemorrhage and haemoglobin is was dropped to 9gm/dl post-operatively. Hence 3 Fetol(iron supplements)and vitamin c were prescribed for her. Importantly, she has no self confidence[it is hyphenated word]  in baby care as well as breast feeding[breastfeeding].She doubts her ability to be a good mother and often discussed regarding offering baby for adoption.

Ms Boyle is single and lives in a rental share flat with other women in Australia since 2015. She has had no contact with the father of the child and her parents. Presently, she receives sole parent benefits. Please note, she feels isolated and unsecure.[insecure]


In this regards regard, It would be greatly appreciated if you could provide interventions advice and assistance for both mother and child in Ms Boyle's and her baby's condition and do the needful. Her address is attached with this letter.[ need to write the address as it is given in the case note.]
