Re:Mr Henry  O'Keefe 

DOB:02 February,1925


Mr O'Keefe ,an 83-year-old man, who is recovering from a  malignant melanoma in his left shoulder. His family requires a home visit as early as possible to assess their needs and to arrange for appropriate further assistance from your team.


On 14 March,2008,Mr O'Keefe was admitted to hospital and underwent large lesion removal. He was discharged after three days and has been assisted in dressing and showering advised to change wound dress prior to removal of sutures on which will be continued until 24th of this month. 


On the subsequent days, we are doing daily home visits,he was assisted with and  wound dressing as well as assisting Mr O'Keefe with his showering. Today's visit,his wound is healing without any complications. 


Mr O'Keefe ,an aged pensioner lives with his wife .They are getting help from their neighbour's ;however, this family has been is unable to manage the house, garden ,shopping and cooking. 


It would be greatly appreciated if you could visit Mr O'Keefe and Mrs Dorothy at house home and observe the requirements. Please make necessary arrangements for the activities of daily living of his family. Their home address is 12 Donaldson Street,Greenslopes ,4121.
