Re:Ms Tabitha Taborlin , 45 years


  • I am  writing to refer Ms.Taborlin ,who has been diagnosed with Type 1 diabetes mellitus, requires stricter glycemic control and possible insulin pump( the purpose of the letter is not that one) .She is being discharged today.

  • Purpose of the letter is deviated 

  • That means, the whole letter went wrong.


Ms Taborlin was admitted on 02 April,2018 with the complaints of Diabetic keto acidosis.Her glucose level were/was recorded  as 530 mmol/ l.She was treated with intravenous fluids and insulin administration .Her HbA1C level is stabilized currently.

Ms.Taborlin is school teacher ,lives alone and dose not exercise at all.(Irrelevent data)Her past medical history reveals essential hypertension, Type 1 diabetes mellitus for which she is non compliance with insulin regimen.Therefore,she had multiple episode of Diabetic keto acidosis.Very importantly,she is under weighed with 48 kgs and BMI of 18.2.She is known case of  diabetes since 7 years for which she was prescribed 45 units of lantus at night and preprandial correction scale with 12 units nutritional balance.However ,she is non compliance with insulin regimen.

In view of the above ,she would need your consultation for diabetic education , glucose testing, insulin administration as well as smoking cessation education.she has been explained about  complications of diabetes.Ms Taborlin would be discharged with educational pamphlet and refill for lantus and Humalog.


  • As we discussed  above the purpose of the letter is to provide educational pamphlets and to refill Humalog and Lancet.

  • So the above paragraph went completely wrong.

Tips to improve 


  • write a simple letter 

  • take the  first 10 minutes  of 40 minute to read ,interpret,organize your case note .

  • Organize  always keep  in mind that the purpose of the letter is important.
