Re:Ms Nicole Boyle,aged 20(both mother and child should be referred)
The purpose of this letter is to[refer] Ms Boyle, who was given gave birth to her first child. She requires a home visit from your facility regarding advice and assistance for child care both mother and child following her discharge today.
Good introduction
On 10 Janurary 2018, Ms Boyle was admitted to hospital and underwent an emergency cesarean [spell check]section due to fetal distress and failure to progress. On 11 January 2018,she delivered a male baby with weight of 4.3kg.
During the postoperative period, her post partum [one word]haemorrhage was minimal of 800ml 1 and wound was dry and clean. 2 She was responded well with treatment; however, her haemoglobin was 90g/l so she was commenced on vitamin c and iron supplements (fefol) Therefore,vitamin c and fefol were commenced . Ms Boyle has less confidence for caring for the baby,bathing and breast feeding[one word].In addition, 3 her baby is sucking well but she prefers to change to bottle feeding.At discharge, baby weight was is 4kg.

Ms Boyle is single and has no contact with family members and father of the child for the last 4months.She feels isolated and insecure as she lives in a rental flat.Presently,she receives sole parent benefits.
Kindly do the needful for her betterment. Please provide support as she has doubt that she is not a good mother and she also verbalized about offering the baby for adoption.(this information should not be here)
