I am writing to refer Mr. Andrew McAllister, a 51-year-old construction supervisor, who presents with bilateral inguinal hernias requiring surgical assessment and intervention.  


Mr. McAllister initially reported mild discomfort and swelling in his right groin during a routine check-up in December 2024. At that time, he was diagnosed with a right inguinal hernia and advised to avoid heavy lifting. Despite implementing some lifestyle changes, including reducing alcohol intake and losing 2 kg, he recently experienced worsening symptoms after lifting a heavy toolbox at work. He now reports persistent right groin pain with marked swelling and a smaller, emerging swelling on the left side.  


On examination, his blood pressure is 145/85, and there is significant swelling and tenderness in the right groin with a smaller protrusion on the left. His past medical history includes Type 2 diabetes (HbA1c: 7.3%) and hypertension, both managed with medication. He is allergic to penicillin (rash, itching).  


Mr. McAllister prefers surgery under local anesthesia, based on advice from a friend, and I have discussed the potential benefits and risks. I would appreciate your evaluation and management of his condition.
