I am writing regarding Jake Peterson, a boarding student of 18 years old who is transfering to your university and will need monitoring due to high levels of anxiety and hair loss that are affecting his development at school.

The patient suffers from alopecia since he was 13 years old with intermittent cycles. It seems to be exacerbated in periods of stress or depression. He was refered to a trichologist who did no established an aetiology but prescribed a steroid cream for Jake. However, he discontinued the medication because of personality changes and sleep loss.

Since December 2018, anxiety has returned along with a pronounced hair loss. In addition, he has lost 6kg over the last 2 months. Although he was recommended to see a doctor, Jake chose to try natural remedies which were not effective and very costly. Also, he has been having problems with forming new peer relationships which is affecting his sports participation and he is increasing alcohol consumption.

Due to the above mentioned, he presented with extreme anxiety during the exam period and it was needed to arrange a separate exam room. 

Please ensure that the patient anxiety and health condition is monitored as he is specially concerned about his transition to university and indepent living.
