Re: Mr Jonathan Thompson ,DOB:01 August 1984 


I am referring Mr Jonathan Thompson into your care for continued wound care and post- discharge monitoring owing to after her as he underwent incision and drainage procedure to treat his perianal abscess.

On 08 November 2020,Mr Thompson was admitted to hospital due to perineal abscess,as a result he underwent surgery under general anaesthesia.Post operatively he was managed by medication and dressing.Mr Thompson wound size is 2 cm long and 1.5 cm width.Presently, all observations are normal and no signs of infection.

Mr Thompson was advised regarding his diet and he was informed not to take ibuprofen as long term.

  • Was he given ibuprofen in the hospital?


Mr Thompson discharge plan is, 1 wound have been cleaned with prontosan , packed with kaltostat and apply tegaderm for sealing. Check his daily wound dressing as exudate is positive and signs of infection. 2 He commenced on tablet metronidazole; 500 mg ; every 8 hours for 7 days. Please refer back him to hospital, if you have any concern regarding healing.He has a review at the surgical outpatient department after 6 weeks.

  • 2 ideally it is not possible for someone to commence his medicine by him or herself
