Dear Community Nurse Supervisor,

Mr. Jones, who has had a surgery of basal cell carcinoma of left calf, is being referred to your centre for follow-up care and management.

Post-operatively, his wound was managed well and there were no complications. However, dressing should be continued. He performs his daily activities alone. He walks with the help of crutches.

He performs exercise regularly. He smokes not more than 10 cigarettes a day and drinks beer daily. However, he does not have any previous hospitalization.

It would be highly appreciated, if you could continue his medications along with dressings and assess need for other services.

In case of emergency, you can contact his ex-wife Mrs. Volga, her address is enclosed.
