Re: Mr. Alfred Billy, 52-year-old

Dear Community Nurse Head,

Mr. Billy, who has had a surgery for basal cell carcinoma of neck, is being referred to your centre for follow-up care and management.

On 21/03/2010, he was admitted to our hospital for a skin biopsy. Biopsy was done and was sent for pathological studies.  There were no complications noted. He dose not have any pain; and now he is perfectly alright. He is on Pandamin forte 500 mg.

He has been consuming liquor for past 30 years and he smokes regularly. Even though, he did not hospitalized previously.

It would be highly appreciated, if you could observe him and continue his medications one more week.
