Dear Nurse,
Ms Osburn, who has attended Newport Community Health Center for the past six years, will require continued care upon moving to Woodville.
Ms Osburn currently lives alone and has been treated for hypertension over the last 10 years and depression for the past two years, for which she currently takes
anti-hypertensives and anti-depressants. Periodically, she has been experiencing problems with regards to self-administration of her medication. In addition, she reports to have a chronic and excessive alcohol intake and admits to drink more when she becomes anxious.
Ms Osburn has agreed with her daughter's suggestion to move to Woodville,
where they may have more regular contact.
Kindly arrange for her to get in touch with a general practitioner for ongoing care. It would be highly appreciated if you could monitor her medication compliance, as well as her alcohol consumption and dietary habits. She would best benefit if you could help her in joining social groups and activities and keep her family in contact with her more often.
