Dear Dr Cassimatis,
Re: Ms Allison Martin, Age 28 years
I am writing to refer this woman, whose clinical features are suggestive of schizophrenia and associated disorders. She would require further assessment and management.
Always write patients name in the introduction
Ms Martin is married, has 2 children aged 4 years and 10 month. She currently lives with her husband’s parents and has a family history of Schizophrenia.
She initially presented on 09/01/10 complaining of poor health, tiredness and lack of motivation at work. She was then counselled on adequate rest and regular exercise.
Three weeks later, her symptoms had worsened with poor concentration and attention to job activities. In addition, anxiety, frequent headaches and poor sleep were reported. Her mood was depressed and she maintained little eye contact. A probable diagnosis of early depression with schizophrenia was made. Therefore, she was commenced on diazepam 10mg nocte.
On today’s visit, Ms Martin’s condition had further deteriorated. She now has visual hallucinations and delusions. Her behavior was bizarre and she has disorganized speech.
In view of the above, Ms Martin would highly benefit from your kind review and further management.
Please do not hesitate to contact me if you have any queries.
Yours sincerely,
Dr Ivan Henjak.
