Re: Mr Alex Maydew,aged 23


 I am writing to refer Mr Alex Maydew,who is recovering from neurological injuries.He requires ongoing  speech therapy, physiotherapy and possible continuation of online studies.

  • Use appropriate verb.not all letters are referral letters. The purpose of the letter is to request to transfer Mr X to a new care facility.

 

Mr Maydew was admitted to the facility with a coma stage following a car accident for the 10 months  ago(giving a wrong meaning). Subsequently, he has made good progress which evidenced by normal brain function; however, he  has loss of speech.

  • Background information should be sufficiently explained.

 

Followingly, physiotherapy has been initiated in which mobility has improved. Mr Maydew showed  signs of depression (reason for this) and his mother concerned about his recovery.


 Mr Alex was a third year physical education student at the university.Currently,he is able to use  computer and showed interest to resume part -time study.His mother and sister are very supportive and caring.

 

In view of the above ,it would be appreciated if you could provide help to restart Mr Alex’s studies coupled with speech therapy and physiotherapy .
