Re: Ms Maryam Hassan , 29 years.
I am writing to refer Ms Hassan whose clinical features and laboratory finding are suggestive of hyperthyroidism. Your further investigations and management would be highly acknowledged.
Ms Hassan presented at our clinic on 27 February,2019 with complaints of a difficulty in sleep and stress at work place . In addition, a slight weight loss associated with headaches and eye dryness were reported. At that time , she was advised to reduce caffeine consumption and follow a proper sleep hygiene .Finish the para here.
A week later, in spite of compliance with her medical advice, she did not show improvement in her sleep pattern . Upon discussion, a sense of heart palpitation was documented. (Include palpation in assessment)On assessment, a blood pressure reading of 132/89 , plus rate of 92 and an ECG reading of 88 were detected were detected. Therefore, a full blood test , TFT as well as 24
ECG recording were recommended.
On today visit’s, Ms Hassan showed no changes in her medical status. Beside that, TSH of 0.3 was record, the other blood studies were normal . It is worth to mention that Ms Hassan has a concern reading her further fertility as she wants to start a family in the next few years.
In terms of Ms Hassan’s medical background , she has a family history of type 2 diabetes mellitus related to her grandmother.
