Re: Ms Dolores Hoffmann DOB : 22 June,1986.
I am writing to refer Ms Hoffmann whose has been diagnosed with reactive depression and anxiety . Your expert management would be highly acknowledged.
Ms Hoffmann has attended our clinic multiple times due to different medical issues, which have been managed accordingly. In September 2004 she visited the clinic with symptoms of URTI along with anxiety regarding the Epstein Bar Virus. She has been advised to take a rest and reassured her regarding the anxiety.
Ms Hoffmann presented at our clinic on 22 November, 2014 with the complaints of difficulty in sleeping associated with a loss of enthusiasm and depressive mood. Upon discussion, a pressure at workplace and memory disturbance were reported. In addition, a lack of appetite together with a loss of libido as well as low self-esteem were recorded . Beside that , Ms Hoffmann has recently break off from her body friend , and now she lives alone. Therefore, temazepam ,20 mg at night was recommended.
Therefore, a referral to the psychiatrist was recommended which she initially refused. In addition, temazepam 20 mg tablets at night were prescribed.
On today’s visit, Ms Hoffmann did not show improvement in her condition ,and she has not been taking her medication as prescribed .
On review today,Ms Hoffman has not filled the temazepam script since she is not keen on drug treatment and agreed to visit you for further investigation and follow-up treatment. Kindly do the needful.
In terms of Ms hoffman's medical background, she has allergy to penicillin, and she is on HSV treatment.
In light of the above, I would be grateful if you could take over the care of Ms hoffman's medical status and provide a further assessment at your facility.
