I am writing to refer Mr Dawson who has been dy with cerebrovascular accident,who requires home visit to improve his mobility.

Mr Dawson fell on 18 may 2008, while descending stairs.he had grazed on left leg knee.General practitioner  advised for daily visit from our facility to care for dressing and assist with showering. The wound redressed and no sign of infection.

Mr Dawson had Cardiovascular accident and recovery is uneventful .He has slight speech impairment and weakness on the left side .He is walking with his wife's support.He lives his own house with his wife on behalf of Mrs Elizabeth.



In view of the above,his wife has requested home visit for assistance to improve his mobility with a walking stick.


It would be greatly appreciated to visit him and provide  care and Management.
