Re : Ms Tabitha Taborlin 

Age : 45 years 


Ms Taborlin has had type 1 diabetes mellitus since 7-year-old and has non-compliance with insulin regimen. She requires further management.

Initially, Ms Taborlin was admitted with diabetic ketoacidosis (DKA)  which was treated with intravenous fluids and insulin. She has been prescribed with 45 units of Lantus at night and 12 units Humalog. During discharge, a stable HbA1C was found. On today's appointment, a  discussion was made regarding the necessity of glucose checking, insulin administration and cessation of smoking as she was noted to have non-compliance with insulin regimen and advised diet. In addition, micro and macro vascular{macrovascular} complication of diabetes was also discussed. Apart from that, she was provided with education pamphlets and refills for Lantus and Humalog.

It would greatly appreciated if you could assist her to maintain stricter glycemic control possibly with an insulin pump.
