Re : Ms Tabitha Taborlin,45 years

I am writing to refer Ms Taborlin who has type 1 diabetes since for 7 years. She  requires, education regarding glycemic control measures and insulin pump usage.

• Wrong purpose

Ms Taborlin is an underweight female currently on 45 units of  Lantus at night and 12 units Humalog .She has had multiple episodes of diabetic ketoacidosis.On 02 april ,2018  she was admitted with glucose level 530 mmo/L and was treated with insulin and iv fluids.

On Today's appointment, she is doing found to be well since discharge. However, she is not compliant with insulin and using insulin and not following the recommended diet. Therefore , she is educated about the necessity of glucose testing, insulin administration and smoking cessation. In addition, discussed her about various complications of diabetes and provided educational pamphlets and refills for insulin injections.

Ms Taborlin is a school teacher living alone in her apartment.she is having a habit of smoking. She has hypertension.
