Re:Ms bethany Tailor,aged 35


This is to inform you about Ms Tailor who has been treated for decompensated schizophrenia. She requires comprehensive care from your facility 1 as she is being following her discharged today.

  • Following her discharged today would be fine

  • 1 She needs care that is not due to her discharge 


On 01 March 2018, Ms Tailor presented with auditory and visual hallucinations,along with delusion., Besides, she had agitation as well as aggressive behaviour. She was treated with anti psychotic medication rispoderone.{stop here}1]During 10th march 2018,Ms tailor was responded well with the treatment and  therapy,her complaints were minimised;hence,she focused on activities of daily living.

Ms tailor is unemployed and getting disable allowance for schizophrenia. She has a habit of polysubstance abuse mainly cocaine and alcohol. She has had hypertension, secondary to fibromuscular dysplasia and primary hypothyroidism; for which, she is on levothyroxine 88mcg daily.

  • The whole para is not needed as it is not relevant. But, you presented it well and you can add as a semi-relevant data.


Kindly visit Ms tailor to her apartment and please do the needfull for her betterment. May I remind you that she is commenced on risperidone 4gram orally every night and 1milligrams twice daily is recommended if she appears agitated or shows any signs of psychosis.
