I am writing to refer Ms Tailor who has been diagnosed with schizophrenia. She requires long -term care from your facility following her discharge today.

On 01 March ,2018,Ms Tailor was admitted to hospital with decompensated schizophrenia. On reassessment,she had visual and auditory hallucinations as well as delusion.Additionally,she was agitated and aggressive;therefore ,she was assessed and commenced on rispoderone 4mg ,which needs to be continued.

  • Use commence in passive need attention 



During hospitalization,Ms Tailor attained good progress. She is  able to minimize delusion and hallucinations ,and 1- also started to focus on her daily living activities. 


  • Avoiding the usage of also would be fine


Ms Tailor is unemployed and is on disability allowance. She has hypertension secondary to fibro muscular dysplasia and primary hypothyroidism for which she has been taking medication. Please note,she has a history of poly substance abuse mainly cocaine and alcohol. 


Pertaining to the above information,it would be greatly appreciated if you could provide  follow-up care for Ms Tailor. Kindly ensure, her medication compliance such as risperidone 4mg at night and 1mg twice daily whenever necessary  for her agitation and psychosis. It is worthy to help her to maintain behavioural control and  if necessary provide therapy.
