Re: Ms Bethany Tailor, aged 35


The purpose of this letter is to update the current health status of Ms Tailor who requires ongoing care and management as she has been diagnosed with schizophrenia. She is being discharged today.


Good Introduction!!


On 1March, 2018, Ms Tailor 1presented to hospital with 2-suggestive symptoms of schizophrenia. On assessment, delusion, visual and auditory hallucination 3were noted for which she has been treated with ripoderone/rispoderone which is to 4be continue after her discharge.

  • 2-it is not suggestive symptoms she is already a patient of schizophrenia. Currently she came with decompensated schizophrenia.

  • There are some data that you missed. Read the case note carefully. Interpret case note organise it.

  • Eg.  Patient admitted with the complaints of visual and auditory hallucinations along with delusions. She had agitation and aggressive behaviour together with thought blocking and latency with responding to internal stimuli. Therefore, she was recommended respoderone 4g at night and an additional dose of 1mg to be given twice daily in case of agitation or psychosis which needs to be monitored.


During hospitalization, Ms Tailor 1)ceased auditory and visual hallucination.she 2)has focus on daily living activities and3) is making good progress.

In view of the above, it would be greatly beneficial if you could provide continuety/continuity of care to for Ms Tailor. Please monitor her medication compliance and signs of psychosis. It is important to maintain behavioural control and provide therapy if possible. Kindly administer anipsychotic/antipsychotic drug 1 milligram twice daily if any agitation or psychosis is present.
