I am writing to refer Ms Tailor who has been diagnosed with Schizophrenia requires on going /ONGOING monitoring and care following by her discharge today.

Ms Tailor was self admitted on 1 March,2018 with the complaints of auditory and visual command hallucinations telling harm herself and visual hallucinations- shadows figures with graining faces.Besides, along with delusions. as personal connections to various political leaders. On assessment, she was agitated and aggressive and was responding to internal stimuli with thought blocking and latency. Therefore, she was commenced with antipsychotic medication risperidone. Fortunately, her symptoms were she ceased reporting auditory and visual hallucinations.

Ms.Tailer lives on disability allowance for schizophrenia. She has a history of polysubstance abuse, predominantly cocaine and alcohol. Her last usage of cocaine was on 28 February 2018.In terms of her past medical history ,she has hypertension secondary to fibromuscular dysplasia.In addition, she has primary hyperparathyroidism for which she is on levethyroxine 88 mcg daily.However,she is not complied with medications.

It would be highly appreciable if you would could moniter/MONITOR  her compliance on medications inorder( two words)to prevent decompensated schizophrenia. Kindly make an arrangement for behavioural therapy if possible. She would be discharged on with Respiridone 4 g daily every at night by mouth as well as Respiridone 1 mg twice daily in case of agitation or psychosis.
