Re: Ms Bethany Tailor

I am writing to refer Ms Tailor who requires monitoring for objective signs of 

psychosis and her medication compliance in view of her schizophrenia at your service. She is being discharged today.

Ms Tailor is an unemployed who has disability allowance for schizophrenia. [1]She has a habit of polysubstance abuse mainly cocaine and alcohol. She experiences hallucination and delusion.[2] She has hypertension secondary to fibromuscular dysplasia and primary hypothyroidism. she takes levothyroxine 88 mcg daily.

Ms Tailor admitted to hospital on 01/03/2018 with decompensated schizophrenia. She was agitated and aggressive [She had agitation and aggressive behaviour]. She responded to internal stimuli with thought blocking and latency.she was treated with risperidone, which needs to be continued if she has agitation or psychosis. Currently, she has made good progress. She is able to minimise delusions and hallucination as well as focus on activities of daily living.


  • Important points not to be avoided


  • You should write current data in detail

  • All of the Advice should write in detail

  • Discharge medicines with dose 

  • Risperidone 4g once at night and risperidone 1 mg prn

  • Ensure medication compliance should not be avoided

  • Insight good and judgement fair


  • Organize in a better manner Eg . write admission details in the first para

And current data in the next para like that ..



It would be greatly appreciated if you could redirect Ms Tailor from delusions[already done ] Kindly help her maintain behavioural control and provide therapy if possible.
