Ms Tailor has been diagnosed with schizophrenia. She requires long -term care at your facility, following her discharge today.

Her medical history revealed,[3] she has hypertension secondary to fibromus and primary hypothyroidism for which she is on levothyroxine,88mcg daily.

 Ms Tailor has a history of alcohol and cocaine abuse. She has used cocaine on 28 February, 2018. She was self-admitted to hospital on 01 March, 2018 with decompensated symptoms of schizophrenia. On assessment, she had delusions, auditory and visual hallucinations. Moreover, she was agitated, aggressive and responded to internal stimuli.

During hospitalizations, Ms Tailor has been recommended with risperidone. In- addition, she was assessed for objective signs of psychosis, redirected her from delusions and ensured her medical compliance. Apart from that she was provided therapy to maintain behavioural control. Currently, she can is able to minimise delusions and focus on daily living activities.   Her insight is good however her judgement is seen as fair are good. Even though, she can be decompensated if not on medications or abusing the substance.

  • Try to minimize minor errors


Upon discharge, Ms Tailor needs to be continued tablet risperidone, 4 grams, every night and was is prescribed 1 milligram, twice daily in case of agitation or psychosis. Please note, she requires follow-up at Proudhurst Mental Health Clinic.
